The forms found on this website are for research purposes only. No parts of the application for the Blue Cards application process are found here or online anywhere. The state application is found on the DOH website.

There are no downloadable State Application forms on this website.

Neither are there any application forms to be sent in by an online mechanism.

The state and I do not accept faxes or pdf copies of the medical forms or application.

Original signatures are required.

Patients unable to come to the office due to disability or illness, may have the required medical office forms sent to them by email or snail mail if it expedites their visit at the office.
